Annual Certification For Dependent Children Ages 19 to 26
The New Hampshire School Health Care Coalition (SchoolCare) provides coverage for qualified dependent children:
- between the ages of 19 and 26,
- who are not covered under any other health benefit plan or policy,
- provided the dependent is unmarried,
- and is a New Hampshire resident or is enrolled as a student at a public or private institution of higher education.
SchoolCare requires an annual certification for all covered dependent children between the ages of 19 and 26. Contrary to past practice, CIGNA will NOT be sending the request for college verification directly to employees. Instead, SchoolCare is sending your Employer a file of all covered dependents requiring annual certification.
You should receive notification from your Employer if you have any dependent children between the ages of 19 and 26. To ensure continued coverage, complete a Dependent Certification Form(s) and submit to your Employer by September 15, 2008. Otherwise, the dependent's coverage will be terminated on September 30, 2008.
Reminder:
- Each child will need a separate form.
- All documentation must be submitted by the Employee, to the Employer who will then submit to SchoolCare for processing. Any documentation that is sent to CIGNA will NOT be processed.
- SchoolCare will review the information and make a determination of coverage eligibility for the dependent child based on the criteria outlined above. If continuation of coverage is not approved, you will be notified accordingly.
